Right clicking the toolbar and selecting "Use Small Size" will not do anything if the text labels are hidden (Show: "Icon Only" is selected). It appears "Use Small Size" is activated no matter if it's selected from the user interface or not.
It makes sense for me to have that activated by default if the users doesn't want to see the text labels, but it's an interface decision against the way other applications work in OS X. Try the same thing with the Finder toolbar. It makes a difference if that is selected or not.
I think the easiest way to solve this is by making it work as expected. A better way to do it would be to disable (make it light grey) the "Use Small Size" menu item if the text labels are hidden.

Well at least can you fix it so that it works as expected and actually move everything a few pixels when selected or not.
Or is this an intended feature?
Anyways, if there is no easy fix for this, just ignore it. Maybe a time for it will come some day
